Save time at your appointment by filling out your new patient paperwork before your visit.
Patients can fill out the new patient questionnaire forms on the MyHSS portal. Patients must be existing patients at HSS in order to access the MyHSS portal. If you are not a pre-existing patient, you must call our office 212-606-1065 (NYC) / 203-705-2113 (CT) to become registered with HSS. Your MyHSS account can be activated online or by receiving an activation code at the time of your first appointment.
