Save time at your appointment by completing your new patient paperwork before you arrive. All forms are submitted through the MyHSS patient portal — a secure online platform managed by Hospital for Special Surgery.


How to Complete Your Forms

  • If you are already registered with HSS, log in to MyHSS at myhss.hss.edu to complete your new patient questionnaire

  • If you are new to HSS, call our office first to register: 212-606-1065 (NYC) or 203-705-2113 (CT)

  • Your MyHSS account can be activated online or with an activation code at your first appointment

  • Submit completed forms before your visit so Dr. McLawhorn's team can review your history in advance


What to Bring to Your First Appointment

  • Insurance card(s) and referral letter (if required)

  • Government-issued photo ID

  • Prior surgical/medical records, office notes, and operative reports

  • Prior imaging on disc or in hard copy, plus associated radiology reports

  • Current medications list and known allergies


Imaging Requirements

Whenever possible, Dr. McLawhorn prefers that patients obtain X-rays at HSS, even if imaging was recently performed elsewhere. HSS radiographs are standardized for joint replacement evaluation and surgical planning, include specialized calibration markers, and capture views that are often not obtained at outside facilities.

Patients are sometimes concerned about repeating recent X-rays. The radiation exposure from orthopedic X-rays is very low, and repeat imaging is routinely obtained to ensure accurate diagnosis and surgical planning. In most cases, these studies are covered by insurance when medically necessary.

If additional imaging is needed, our office will coordinate a separate radiology appointment prior to your visit. Please plan to arrive at least 15–30 minutes before your first scheduled appointment to allow time for registration.
